Work Search Log - Unemployment Insurance

IMPORTANT INFORMATION

The Work Search Log is a convenient way to keep a detailed record of your weekly job contacts while filing for unemployment insurance benefits. The form can be printed or saved to your computer so it is easily accessible. As you have been told, you must make at least two job contacts per week and include the following information:

  • Date contacted
  • Contact information (address, telephone number, website information, etc.)
  • How you contacted the employer (in-person, telephone, internet, etc.)
  • Person contacted (if available)
  • Type of work sought
  • Did you apply for a job, if so how (in person, internet, mail, filled out application, sent resume, etc.)
  • If internet application completed, confirmation number
  • Results of application (pending, not hiring, will call, not accepting application, etc.)

Remember, your work search contacts are subject to verification by the Division of Unemployment Insurance. You must keep a copy of your work search records for one year after the contacts were made.

Firefox for PC:

  1. Download Adobe Acrobat for free if you have not already done so.
  2. Click on the Tools menu at the top left portion of your Firefox browser, and then select Options.
  3. Select the Applications panel.
  4. Find Portable Document Format (PDF) in the list and click on it to select it.
  5. Click on the drop down arrow in the Action column for the PDF entry and select Use Adobe Reader.
  6. Click OK to close the Options window.
  7. Click on the Work Search Log (PDF) to open the file.

Chrome for PC:

  1. Download Adobe Acrobat for free if you have not already done so.
  2. Type “chrome:plugins” into the address bar of your Chrome browser.
  3. Find Adobe reader plugin on the list.
  4. Click Enable to activate the plugin.
  5. Disable Google PDF viewer.
  6. Click on the Work Search Log (PDF) to open the file.

The Preview App is the default application in MAC used to open PDF files. However, the Work Search Log is a fillable PDF file. Because of this functionality, the PDF file needs to be opened using Adobe Reader. If you are experiencing difficulty opening this PDF file in Adobe Reader using either Safari or Chrome in a MAC, please follow the instructions below.

* Please note that the instructions will NOT deactivate/disable the Preview App. Adobe Reader will only be used to open the Work Search Log PDF file.

Safari for MAC:

  1. Download Adobe Acrobat for free if you have not already done so.
  2. Click on the Work Search Log (PDF) to open the file.
  3. A document will open but this does not contain the proper contents of the document.
  4. To properly view the contents of the document, please follow these instructions:
    • Click on File Menu
    • Click Export as PDF
    • A dialog box will appear.
    • Type a name for the document and indicate the location in your MAC where you'd like to Save the document.
    • Click Save.
    • Navigate to the location where you saved the document.
    • Right-click on the document.
    • Click Open With.
    • Click Adobe Acrobat Reader DC.App

Chrome for MAC:

  1. Download Adobe Acrobat for free if you have not already done so.
  2. Click on the Work Search Log (PDF) to open the file.
  3. A document will open but this does not contain the proper contents of the document.
  4. To properly view the contents of the document, please follow these instructions:
    • Click on the Save Icon (disk) on the floating toolbar. (The floating toolbar is located on top of the document, the Save icon is the third icon.)
    • A dialog box will appear.
    • Type a name for the document and indicate the location in your MAC where you'd like to Save the document.
    • Click Save.
    • Navigate to the location where you saved the document.
    • Right-click on the document.
    • Click Open With.
    • Click Adobe Acrobat Reader DC.App
