Below is a list of commonly used forms necessary for the registration and inspection process of new and existing amusement attractions (available in Word format, download Word viewer for free and Adobe Acrobat format, download Adobe Acrobat for free).

• Amusement Attraction Inspection Request Form (Word)
  Use this form to request an inspection of an existing registered amusement attraction
• Investigation Form (Excel)
  Use this form to report an accident, incident, or complaint.
• New Mechanical Amusement Attraction Registration Form (Word)
  Use this form to request an inspection of a new NON-INFLATABLE amusement attraction.
• New Inflatable Amusement Attraction Registration Form (Word)
  Use this form to request an inspection of a new INFLATABLE amusement attraction.
• Owner Incident Form (Excel)
  Amusement Attraction Owners: Use this form to log Amusement Attraction incidents that occur during a calendar year. This form must be finalized and certified by a company executive by January 15 of the following calendar year.
• Owners Inflatable Checklist (daily pre-opening checklist) (Word)
  This is a daily pre-opening checklist that owners should use to identify unsafe conditions. It is the owner's obligation to correct any identified deficiencies before operation.
• Witness Statement Form (Word)
  Use this form to describe what was seen prior, during, and after an accident, incident, or complaint.
