Name/Address Change - Individual Tax Preparers

Registered individual tax preparers are responsible to notify the Board of any name change or change of mailing/e-mail address.

Address changes may be made online using your registration number and password:

Name changes must be made by mail using using the following form:

E-mail completed forms to DLOPLTaxPrep-LABOR@maryland.gov.

There is NO fee for address/name changes.

Failure to notify the Board of address changes in a timely manner can prevent registration renewal notification. A $120 late fee will be charged when a registration is not renewed by the expiration date.
