Information & Documents Needed for Registration - Household Goods Movers Registration

Household goods movers will be asked to provide the following information or documentation with their application:

Information

  1. Applicant’s Name(Must be the Owner, Chief Executive Officer, or Resident Agent)
  2. Corporate/Company Name
  3. All trade names under which the applicant intends to provide household goods moving services
  4. Names of all persons with at least 25% ownership in the applicant’s business. NOTE: If a person identified with at least 25% ownership has previously applied for registration for another entity,  you will need to provide identification of that person and the disposition of the application for the other entity.
  5. Federal Employee Identification Number (FEIN)
  6. Name of the entity’s resident agent in the State, if any
  7. U.S. Dept. of Transportation or State Dept. of Transportation number
  8. An email address and an alternate email address
  9. Business address or address of the resident agent if the business is not located in MD.

Documents

  1. Insurance carrier and policy number showing liability and cargo coverage with the minimum standards in 49 C.F.R. Part 387.303
  2. Proof of workers’ compensation coverage for all covered employees
A certificate of status, issued by the Maryland Department of Assessment and Taxation, or its equivalent, indicating the business is in good standing NOTE: this certificate must be dated within 30 days before the date of the application
