Name/Address Change - Cosmetologists

Licensed Cosmetologists are responsible to notify the Board of any name change or change of mailing/e-mail address. Changes can be made:

Online using your registration number and password

Name/Address Change – Individuals

Name Change – Business Entities

OR

Via e-mail to barbers.cos@maryland.gov

There is NO fee for address/name changes.

Name Changes — Individuals
If you change your name, you must provide a copy of the appropriate court document or marriage license and e-mail/fax it to the Board. E-mail the document(s) to the address above, or fax them to 410-962-8483 .

Address Changes — Business Entities
Salon owners must apply for a NEW permit through the Original Shop Permit application if the shop changes ownership or the shop location has changed.

The following must be provided before approval of your name change.

  1. A photo of the outside signage showing proof of the name change
  2. A notarized letter including the information below:
  • The current shop name of your salon/shop
  • The address of your salon/shop
  • Salon/Barbershop license number (5-6 digits)
  • New shop name
  • Beginning "as of" date
  • Owner(s) name
  • Owner(s) phone number and email
  • Signature of the Owner(s)

This letter may be sent via email; Barbers.cos@maryland.gov. Once we have documentation, we will update your file in our database, indicating the details requested.

If you are selling the shop/salon to a new owner, DO NOT simply give them your license. They must apply for the owner's permit themselves and may contact the Board for assistance. You may provide them with the Use and Occupancy Permit for the location if there have been no major changes/renovations to the structure.
